Operational Incident Diagnostic

Start with the record you already have.

One approved historical incident export goes in. What comes back is a traceable view of the conditions that keep recurring, where the record is too thin to support a conclusion, and controls your team may want to evaluate.

How the engagement works

01

Scope

You provide a representative sample. We confirm what the record contains, what it does not, and whether the engagement is a fit.

02

Map

chatIR maps the approved export into consistent operational conditions without replacing the source record.

03

Analyze

The review looks for recurrence and interaction across records. Material findings stay tied to their supporting populations.

04

Review

You receive the diagnostic and a live working session covering the evidence, its limits, and controls worth evaluating.

What we need

A useful historical export

CSV or Excel is fine. Your existing column names are fine. Narrative detail matters more than a perfect template. A sample review establishes whether the record has enough depth and volume to proceed.

Current data boundary

Deidentified operational data only

chatIR does not currently accept PHI. We are building the contractual and technical safeguards needed to support it, including BAAs with applicable vendors.

Scope is confirmed before money or data changes hands.

Larger archives, multiple source systems, and substantial remediation can change the price or schedule. If the supplied record cannot support useful work, we stop before the engagement begins.
